Job Purpose

To provide personal care and assistance with activities of daily living to clients in their place of residence in order to help the client achieve optimum level of functioning and self-management.

Duties and Responsibilities

Performs only under the supervision of an RN or Therapist who has provided written instructions for client care.

Performs total care or assists clients in all activities of daily living and personal care such as, but not limited to, bathing, grooming, linen changes, and personal laundry.

Assists clients with transfers, ambulation, assigned exercises, and toileting, including the use of a bedpan or commode.

Prepares nutritious meals within the client’s diet and assists with eating when necessary.

Performs light housekeeping chores, which facilitates client self-care in the home setting.

Answers client’s calls and attends to the client’s requests promptly.

Meets the safety needs of the client and uses adaptive equipment safely and appropriately.

Observes client’s general condition, takes and records accurately vital signs, and reports changes promptly to the nurse.

Accurately documents dates, visits, and completed tasks and carries out all requested assignments.

Reminds and assists patients in taking medications.

Recognizes emergencies and utilizes emergency procedures.

Submits paperwork in a timely manner according to agency policy and procedure.

Qualifications

Completion of the 9th grade; high school diploma preferred.

Successful completion of a State-established or approved training program that meets the requirements of Standard 484.36(a) and a competency evaluation program or State licensure program that meets the requirements of Standard 484.36(b) or (e).

Completion of one year’s experience as a Nurse’s Aide in a hospital or health-related facility.

Recent completion of training or experience within the last 24 months as a Home Health Aide.

Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the client, client’s family, and the personnel of the agency.

Demonstrates a sympathetic attitude toward the care of the sick, ability to read, write, and carry out directions, and the ability to deal effectively with the demands of the job.
